Today we return to the foundation of everything I teach: how to prescribe bioidentical hormone replacement safely and effectively. The literature supports individualized protocols — not cookbook medicine. Before you write the first prescription, you need a clear indication, baseline labs, symptom mapping, and a monitoring plan. Initiation should be conservative. We titrate to clinical response and validated labs, not population averages. Patients are not statistics. Monitoring is where many clinicians fail. Hormone therapy is not fire-and-forget. You follow symptoms, blood pressure, metabolic markers, and hormone levels on a schedule appropriate to the formulation and patient risk profile. The goal is not to hit a number on a lab report. The goal is to restore physiologic function while minimizing avoidable risk. That is BHRT done responsibly.
