Complex cases are where hormone medicine becomes real clinical medicine. When a patient plateaus, worsens, or develops unexpected labs, the reflex should not be to stop therapy. The reflex should be to re-diagnose: absorption, conversion, compliance, competing diagnoses, drug interactions. I teach a structured approach: confirm the story, repeat critical labs, review formulation and timing, then adjust one variable at a time. Adverse effects are information. Erythema, fluid retention, mood changes, sleep disruption — each points to a mechanistic question about estrogen, progesterone, testosterone, thyroid, or metabolic context. Document your reasoning. Complex case excellence is not guessing; it is disciplined iteration with the patient in the loop.
