Welcome back. Today I want to address one of the most misunderstood monitoring parameters in testosterone replacement therapy: hematocrit. Physicians have been taught for decades that rising hematocrit automatically means increased cardiovascular thrombotic risk. That assumption deserves careful scrutiny. In appropriately selected hypogonadal men, modest hematocrit elevation often reflects improved red cell mass and oxygen-carrying capacity rather than pathologic polycythemia. The key is context: baseline cardiovascular status, symptoms, dosing, and trajectory. I do not ignore hematocrit. I monitor it. But I also do not reflexively discontinue therapy at arbitrary cutoffs without evaluating the whole patient. When hematocrit rises above individualized thresholds, we discuss phlebotomy, dose adjustment, and hydration before abandoning a therapy that may be restoring quality of life. The literature on testosterone and cardiovascular outcomes has evolved. Contemporary reviews suggest that blanket fear of testosterone based on outdated observational signals is not supported when therapy is appropriately indicated and monitored. Remember: our job is not to practice defensive medicine based on myth. Our job is to interpret evidence, monitor intelligently, and individualize care.
