Women's hormone therapy requires nuance that guideline summaries often flatten. Perimenopause is not a single lab pattern. Symptoms precede textbook hormone changes. Our job is to listen, measure thoughtfully, and individualize. Estrogen and progesterone work as a system. Progesterone is not an afterthought — it matters for sleep, mood, breast tissue signaling, and endometrial protection depending on the regimen. I use bioidentical hormones because the physiologic rationale and clinical experience align with how I want to practice. The evidence debate around cancer and cardiovascular risk must be taught honestly, with study design in view. Initiate low, monitor symptoms and labs, and revisit risk annually. Fear-based refusal of HRT has harmed symptomatic women as much as careless prescribing ever did.
