Let's talk about testosterone replacement therapy and cardiovascular disease. This remains one of the most controversial areas in hormone medicine. Early observational studies created fear. Newer trials and meta-analyses have challenged simplistic conclusions. For hypogonadal men with appropriate indications, the question is not whether testosterone is universally safe or universally dangerous. The question is which patients benefit, how we monitor, and what outcomes matter. Cardiovascular risk assessment should precede therapy. Blood pressure, lipids, glucose, thrombotic history, and symptom burden all belong in the conversation. I teach physicians to separate population-level statistics from the man sitting in front of them. Evidence synthesis helps, but clinical judgment still matters. When patients ask about heart attack risk, we cite the best available studies, explain limitations, and document shared decision-making. That is responsible hormone medicine.
